HealthCare Financial in Office to patients without Dental Insurance
Alvarez Dental participates with most major insurances. Although we do not accept all types of insurance, our practice will complete and electronically submit dental insurance forms to the insurance company to achieve the maximum reimbursement to which our patients are entitled as quickly as possible. Unfortunately, some of the services that you may need or want will not be covered by your dental insurance.

Our goal is to help our patients achieve and maintain optimal dental care, mainly with patients without dental benefits. In order to relieve our patients' a full dental procedures payment, make affordable quality dental care and maximize their ability to complete comprehensive, elective dentistry, Alvarez Dental offers in-office dental financing through BondLife Program
.
1- Two cleanings per year – Additionals, 50% off.
2- Fluoride Treatment
3- X-Rays as needed
4- Examinations (2 per benefit year)
5- One Emergency Exam (if necessary)
6- 30% off all other dental services provided in our office.
7- Deep Cleaning in one quadrant. (if needed)
May not be combined with insurance plans, discounts or promotions – This is not an insurance plan, but a prepaid payment arrangement provided by Alvarez Dental, PC and is not transferable to any other dental office or dental specialist – All accounts must be current before scheduling an appointment – Payment for services are due date of service – Membership is for you and not transferable – Benefits provided for one year from the date the program is purchased – No refunds will be provided for dues of the program annual cost under any circumstances, including failure to schedule and maintain appointments. No deductible, no waiting period, no exclusions.
